The Siskiyou County HIV/AIDS Foundation provides access to health care and support services for individuals who are HIV positive.  The main purpose is to provide access to the needed services to keep each HIV positive individual medically, physically and emotionally stable.  This is provided through ongoing case management by a team consisting of a social worker, a registered nurse, the client’s medical doctor and an infectious disease specialist.

Services that are either provided or services that the case manager helps the client to access are:

  • Medical care, Laboratory and Diagnostic services
  • Bimonthly HIV clinics with an infectious disease specialist
  • Help with accessing medications
  • Dental and Vision care
  • Mental Health and Substance Abuse counseling or referral
  • Case Management
  • Benefits Planning
  • Health Education and Risk Reduction Information
  • Legal Services Referrals
  • Nutrition Assessments and Supplements
  • Transportation
  • Necessities Assistance including Food, Housing and help with Utilities
